Mental Health Case Management

Personalized, collaborative, and flexible client-centered services engage, support, and integrate individuals in their preferred settings, such as at home. Case managers play a pivotal role in connecting individuals with resources that promote independence. Our staff assists in managing illnesses, increasing life skills, and improving a sense of well-being, ultimately supporting wellness and recovery by addressing various critical personal concerns. JFS staff work in partnership with primary care physicians, involve family members, and offer sensitive and culturally competent treatment plans. Our continuous care services are especially beneficial for individuals who are transitioning from state psychiatric hospitals.  By coordinating services across different sectors, case managers ensure that clients receive comprehensive and cohesive support that addresses their physical, mental, and social needs.

Our services are available on-call 24 hours a day, 7 days a week, and bilingual case managers are also available.

Counseling

Counseling effectively enhances life quality by teaching stress management, resolving personal issues, and modifying behaviors. We offer counseling services in our offices in Atlantic City, Margate, and Cape May Courthouse. Additionally, in appropriate circumstances, JFS provides counseling in-home or via telehealth.

ICMS services are specifically for individuals transitioning from state and local psychiatric centers or crisis screening centers (PIP) or are at-risk of hospitalization who are capable of living in the community with appropriate support.

PATH focuses on actively reaching out to individuals experiencing or at risk of homelessness. Our dedicated team conducts regular outreach in common public areas, ensuring a strong presence within the community. Ongoing case management supports these individuals by helping them access essential services and resources.

In partnership with AtlantiCare Behavioral Health and the Atlantic City Rescue Mission, JFS provides case management services to a five-bed transitional residential unit that helps program participants in psychiatric crisis avoid psychiatric hospitalization. An individual’s case manager supports them during their tenure in the unit and links them to essential community services. A community linkage/re-entry group is also provided by the case manager as part of the program’s partial care programming.

The most common reasons people don’t seek care are stigma, cost, misinformation, and lack of information about mental health care.

JFS is a partner in the “Stop the Stigma” initiative aimed at reducing and ultimately eliminating the stigma (e.g.,negative stereotypes, prejudices, and discrimination, etc.) associated with mental illness and addiction so individuals feel supported by the community.
